OIR-B1-1802, formally the "Uniform Mitigation Verification Inspection Form," is the single document Florida's Office of Insurance Regulation adopted so every insurer would score wind mitigation features the same way. It has seven questions, and your discount is built from the answers. Question 1: Building code compliance

The inspector confirms whether your home was permitted under the 2001 Florida Building Code or later (permit application dated on or after March 1, 2002), or, for Miami-Dade and Broward only, the 1994 South Florida Building Code (permit dated on or after September 1, 1994). This single answer is the biggest lever on the form: per the Florida DFS consumer guide, homes that clear this bar are "automatically eligible for a minimum 68% discount on the windstorm coverage of the premium," before any other feature is even scored, because the code itself already mandates upgraded roof covering, roof attachment, and roof-to-wall connections. Question 2: Roof covering

The inspector records every roof covering type on the home (shingle, tile, metal, built-up, membrane) and checks whether each has a valid Florida Building Code or Miami-Dade product approval number, a qualifying permit date, or was originally installed in 2004 or later. A roof covering with no documentation at all scores at the bottom of this question, which is why keeping your roofing permit or product-approval paperwork matters even years after the job is done.

Question 3: Roof-deck attachment

This is scored from inside the attic. The form grades the weakest fastening method found anywhere on the roof deck, from staples or widely spaced nails at the low end up to 8d nails spaced 6 inches apart in the field, or reinforced concrete decking, at the top. "No attic access" is its own answer on the form, which is one reason inspectors need attic access to give you full credit rather than the default lowest tier.

Question 4: Roof-to-wall attachment

Also scored from the attic, this question grades how the roof trusses are tied to the top of the walls, from toe nails (weakest) through metal clips, single wraps, and double wraps, up to a structural connection like anchor bolts (strongest). The form specifically grades the weakest connection type found anywhere on the roof, not the average, so a single toe-nailed truss can cap the whole home's score on this question.

Question 5: Roof geometry (shape)

A hip roof, sloped on all four sides with no more than 10% of the roof perimeter in another shape, is the strongest-scoring roof geometry on the form. The Florida DFS guide describes it plainly: a hip roof "is the most wind resistant roof shape." Gable roofs and other shapes are graded lower because their flat end walls catch more wind pressure.

Question 6: Secondary water resistance (SWR)

SWR is an extra waterproof layer, a self-adhering membrane or foam barrier applied directly to the roof deck, that keeps water out if the roof covering itself is blown off in a storm. Standard felt underlayment does not qualify; the form is explicit that only a sealed-deck-style barrier counts.

Question 7: Opening protection

This is the most detailed question on the form. The inspector rates every window, entry door, garage door, and skylight against a lettered protection scale, from full cyclic-pressure and large-missile-impact testing (the top tier) down to no protection at all. Critically, the form grades the weakest glazed opening on the home for the main score: a single unprotected bathroom window or an unprotected garage door can drop the whole house out of the top credit tier, which is why "protect every opening, not just the big ones" is the single most common mistake homeowners make before an inspection.

Who can sign the form, and how long is it valid?

Only home inspectors licensed under Florida Statute 468.8314 with hurricane-mitigation training, certified building code inspectors, licensed contractors, professional engineers, or licensed architects may sign, per Florida Statute 627.711(2)(a). Once signed, the form is valid for up to five years unless the structure materially changes or an inaccuracy is found. Knowingly submitting a false form is a criminal offense under Florida Statute 627.711(4) through (7), a line printed directly on the form itself.
